Information and Referrals

Referrals are received from the older adults, family members, care-givers, agencies and medical professionals. These referrals usually inform the Outreach Coordinator of a need. The coordinator then meets the older adult in need and assesses their appropriateness as an Outreach Program client.

The client is referred to appropriate agencies outside the Golden Circle, for example Lifeline or Meals-on-Wheels, or they are referred internally to our Volunteer Program or Home Maintenance Department. The Volunteer Program includes rides to Seniors Lunch Express, medical appointments and grocery delivery.
